Delta Dental can administer a self-funded plan with full access to Delta
Dental's premier network. Pooled Small Group Plans (for employers with 5 to 49 eligible employees)
Under Delta Dental of South Dakota's pooled group plans the employer pays
100% of the single cost as an employee benefit for all eligible employees.

Delta Dental of South Dakota offers employer-sponsored dental benefits with
minimal or no employer contribution.
Voluntary Group Plan Application
The Voluntary I Dental Plan requires that the employer enroll at least 35% of eligible employees(minimum of three primary enrollees), and either: 1. Pay at least 25% of the single premium for all employees electing coverage; or, 2. Allow employees to pre-tax their dental premium. Benefit Design and Underwriting Guidelines The Voluntary II Dental Plan requires that the employer enroll at least 50% of eligible employees(minimum of ten primary enrollees), and either: 1. Pay at least 25% of the single premium for all employees electing coverage; or, 2. Allow employees to pre-tax their dental premium. Benefit Design and Underwriting Guidelines The No Minimum Voluntary Dental Plan
requires no employer contribution and there is no minimum percentage of
employee participation (there must be a minimum of two employees).
